Integrating Palliative Care into the PCMH-N Model Training

A team-based care approach to integrating palliative care into primary care

To embed the principles of palliative care utilizing an interdisciplinary, team-based approach to delivering care for patients with chronic medical or life-limiting illness.

Forty-five million Americans are living with one or more chronic conditions that limit personal function and are likely to worsen rather than get better. In Michigan, over 62,000 residents die of chronic disease each year. Ninety percent of the nation’s $3.5 trillion in annual health expenditures are for those with chronic and mental health conditions and the total cost of chronic disease in Michigan alone is projected to be $1.3 trillion in 2016-2030.

The current palliative care workforce is expected to decline drastically during the next 25 years, as demand rises. Sixty-two percent of hospice and palliative care clinicians also report experiencing burnout. There simply are not enough highly trained palliative care specialist providers to meet the growing demand, especially in outpatient and rural settings.

The Integrating Palliative Care into the PCMH-N Model Initiative Training is designed to establish a framework to support early identification and management for those needing palliative care services that provide:

  • Early introduction of primary palliative care services by the interdisciplinary team within their scope of practice
  • Earlier referrals or consultations with specialists based on need and complexity, not prognosis
  • Support of palliative care specialists through ongoing palliative care management by the multidisciplinary care team
  • Increasing access to specialty palliative care services for those that need it the most, while reducing provider burden.

This is a joint training collaboration between Michigan Center for Clinical Systems Improvement and Practice Transformation Institute (PTI).

Mi-CCSI and PTI have been contracted by Blue Cross Blue Shield of Michigan as training entities for this project.

These trainings are open only to practices and their physician organizations (PO) participating in the BCBSM Integrating Palliative Care into the PCMH-N Model Initiative.

Practice Teams participating in this initiative must complete the following base training which will include a combination of live and on-demand learning activities.  The topics are listed below:

  5. Care of the Patient Nearing the End of Life – Carol F. Robinson, DNP, MS, BSN, RN, CHPN®

Guidelines for participation:

  1. PO and each practice unit completes a practice assessment (one for each practice interested).
  2. Send completed practice assessment to Sue Vos at sue.vos@miccsi.org.
  3. Schedule a meeting with Sue Vos at sue.vos@miccsi.org for the practice and PO representatives to review the assessment.
  4. Practice attendees register for each applicable training date.

Attendees of this training include:  Primary Care or provider champion, Care Manager (nurse or social worker), and other members of the practice team.

Cost

FREE for members; FREE for non-members

This training is provided at no cost to the participant through a contract with BCBSM.

Statement of Accreditation

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Michigan State Medical Society (MSMS) through the joint providership of Practice Transformation Institute and MI-CCSI. Practice Transformation Institute is accredited by the MSMS to provide continuing medical education for physicians.
