An evidence and team-based care approach to integrating behavioral health into primary care
The CoCM Base Training is required for adult and adolescent practices wanting to participate in the CoCM program. During this multi-day, live, virtual base CoCM training course, the learner will gain knowledge and skills to implement a collaborative care model (CoCM) that includes a primary care or pediatric provider, behavioral health care manager, and psychiatric consultant functioning as a team, using a shared care plan, to meet the needs of patients with moderate to severe depression and optional anxiety.
- Discuss the process of the CoCM including patient identification, screening, assessment, care planning, systematic case review, interventions, monitoring, and case closure.
- Review and define the roles and responsibilities of the CoCM treatment team.
- Explain the key role and responsibilities of the Behavioral Health Care Manager (BHCM) including care coordination with other specialties and community agencies.
- Discuss the technologies involved in the CoCM process and their application toward population health and treat-to-target goals.
- Review how to bill CoCM services using the collaborative care codes.
- Review and discuss the skills and principles of motivational interviewing, behavioral activation, and problem solving treatment as it applies to patient engagement and assessing risk and safety.
- Review common anti-depressant and anti-anxiety medications and their relative roles in therapy.
- Review implementation strategies beyond initial training.
It is strongly encouraged that the BHCM complete the Team-Based Care Training & Patient Engagement Training prior to attending this training.
This training was co-developed by the Michigan Center for Clinical Systems Improvement (Mi-CCSI), Michigan Collaborative Care Implementation Support Team (MCCIST), and Michigan Institute for Care Management Transformation (MICMT). Mi-CCSI ensures trainers are well versed in the concepts and have extensive experience to offer insight and application of the skills, tools, and information covered.
Mi-CCSI and MCCIST have been contracted by Blue Cross Blue Shield of Michigan as training entities for this project.
These trainings are open only to practices and their physician organizations (PO) participating in the BCBSM CoCM program.

Day 1 8:00 am-4:00 pm | Attendees: provider champions, psychiatrist, behavioral health care managers (BHCM), process improvement specialists, practice managers, PO representatives (Providers & psychiatrists will attend the morning session only).

Guidelines for participation:
- PO and each practice unit completes a practice assessment (one for each practice interested).
- Send completed practice assessment to Sue Vos at sue.vos@miccsi.org.
- Schedule a meeting with Sue Vos at sue.vos@miccsi.org for the practice and PO representatives to review the assessment.
- Practice attendees register for each applicable training date.
Audience
This course is intended only for those physician organizations and primary care practices participating in the BCBSM CoCM Program. If you have questions about eligibility requirements, please contact Sue Vos at sue.vos@miccsi.org.
Attendees of this training include: Primary Care or Pediatric provider champion, psychiatric consultant, Behavioral Health Care Manager (nurse or social worker), and up to three other staff per practice (e.g., clinical supervisor, practice manager, billing specialist, etc.)
FACULTY
Sue Vos, BSN, RN, CCMSusan Vos is the Program Director at the Michigan Center for Clinical Systems Improvement. She has experience in working with multi-disciplinary teams to provide care for complex patients, extensive knowledge and expertise in practice transformation, developing and implementing care management curricula based on evidence-based guidelines, training of care manager and other clinical practice staff, implementing care models, and driving quality improvement strategies. She is the current president of the Case Management Society of America: Grand Rapids/Kalamazoo Chapter. Susan led the organizational team in the COMPASS (Care Of Mental, Physical And Substance-use Syndromes) Initiative funded by the Centers for Medicare & Medicaid Services (CMS) / Center for Medicare & Medicaid Innovation (CMMI), which evaluated the large-scale implementation of the TEAMcare model treating patients with depression and comorbid diabetes and/or cardiovascular disease. The initiative was implemented in 18 medical groups and 172 clinics across eight states.
Robin Schreur, BS, RN, CCMRobin Schreur is a trainer for the Michigan Center for Clinical Systems Improvement (Mi-CCSI). She served as the Clinical Lead Care Manager in Ambulatory Care at Spectrum Health from 2012 until 2020. Her aim is to instill the importance of patient-centered care across the continuum of care. Robin has worked in a variety of settings including insurance-based and primary care practices, and as a behavioral health nurse at an inpatient facility. She has extensive experience in training and onboarding hundreds of clinicians in care management roles and motivational interviewing.
Laura Saunders, MSSWLaura Saunders is the Wisconsin State Project Manager for the Great Lakes Addiction, Mental Health, and Prevention Technology Transfer Centers. Since 2001, Laura has provided SBIRT and Motivational Interviewing training to physicians, nurses, medical students, psychologists, specialty addiction treatment providers, social workers, physical therapists, health educators, and staff who work in correctional settings. She has provided feedback and coaching to hundreds of social workers, correctional staff, and other human service providers who are interested in using evidence-based practices with fidelity. Laura joined the international group of Motivational Interviewing Network of Trainers (MINT) in 2006 (Sophia, Bulgaria) and is an active member of the Wisconsin MINT group.
Mark D. Williams, M.D.Mark Williams received his medical degree at the University of Minnesota and his residency training in Psychiatry at Mayo Clinic. He is an Associate Professor of Psychiatry at Mayo and has authored over 50 articles and book chapters mostly on topics related to integrated behavioral health. He has been a leader in the translation of evidence-based models of care coordination into primary care practices for adult and child depression, and patients with complex diseases both at Mayo and as a part of state and national initiatives. He currently leads the research group within the division of Integrated Behavioral Health at Mayo in Rochester, Minnesota.

This live series activity, Preparing to Implement Collaborative Care, has been reviewed and is acceptable for credit by the American Academy of Family Physicians. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Approved for (1 credit per session ) AAFP (Prescribed) credits.
AMA/AAFP Equivalency: AAFP Prescribed credit is accepted by the American Medical Association as equivalent to AMA PRA Category 1 credit(s)™ toward the AMA Physician’s Recognition Award. When applying for the AMA PRA, Prescribed credit earned must be reported as Prescribed, not as Category 1.
