The Collaborative Care Model (CoCM) Live Base Training Resources

The CoCM Base Training is required for adult and adolescent practices wanting to participate in the CoCM program.  During this multi-day, live, virtual base CoCM training course, the learner will gain knowledge and skills to implement a collaborative care model (CoCM) that includes a primary care or pediatric provider, behavioral health care manager, and psychiatric consultant functioning as a team, using a shared care plan, to meet the needs of patients with moderate to severe depression and optional anxiety. This base CoCM training provides an overview of the Collaborative Care model, patient identification strategies, the roles of the care team, billing codes, and implementation strategies.

Intended audience

This session is intended for primary care and pediatric provider champions, psychiatrist, behavioral health care managers (BHCM), process improvement specialists, practice managers, PO representatives (Providers & psychiatrists are required to attend the morning session of Day One only).
